If you have a Texas driver license or identification (ID) card and you have recently moved into a new address, you are required to change your driver license and/or ID card within 30 days.
There are three ways to report a change of address and request a replacement driver license: online, mail, or in-person.
This article will show you how to request a replacement driver license through the mail.
What is Form DL-64?
Form DL-64 is the application for change of address on valid texas driver license & ID card. This form is used to report a change of address and request an updated driver license and/or ID card.
Form DL-64 is only used for reporting a change of address and requesting an updated driver license and/or ID card.
Reporting a change in any information other than a change of address will require an in-person visit to a local driver license office.
You can only report a change in address through the mail if:
- Your driver license has not yet expired;
- You have a non-commercial driver license;
- You are at least 18 years old;
- Your driver license is currently valid;
- Your Social Security Number if already on file;and
- You are a US citizen.
How to Fill Out DL-64?
When providing your information, write at most one letter, number, or period for each box that is provided. Do not write two or more letters, numbers, or periods in a single box. You may leave a box blank when necessary.
Begin providing your information by writing your answer from the leftmost box and continue writing to the right side until you have either completed your answer or reached the rightmost box.
If the information you are to provide requires a space, leave the corresponding box blank.
Write the letters of your answer in capital letters.
Remember to answer the form completely and accurately.
If you are a US citizen, check “Yes.” Otherwise, check “No” and apply to your local driver license office to change your address and obtain a replacement driver license and/or ID card.
Driver License Number
If you are requesting a replacement driver license, provide your current driver license number.
ID Card Number
If you are requesting a replacement ID card, provide your current ID card number.
Date of Birth
Provide your date of birth.
Social Security Number (SSN)
Provide your SSN.
Suffix (if applicable)
Provide your suffix, if applicable.
Provide your last name.
Provide your first name.
Provide your middle name or your maiden name.
Provide the address of your current residence. Include the street address, city, county, state, and zip code. The address you are to provide cannot be a post office box or a business address.
Provide your email address.
Provide the address where you wish your replacement driver license or ID card to be mailed. Include the street address, city, county, state, and zip code.
If you want to support the Glenda Dawson Donate Life Texas donor registry, indicate a donation amount of $1 or more and check “Yes.” Otherwise, check “No.”
If you would like to register as an organ donor, check “Yes.” Otherwise, check “No.”
If you are a US citizen and would like to register to vote or if already registered, would like to update your voter information, check “Yes.” Otherwise, check “No.”
If you would like to provide two emergency contacts who are to be contacted in the event of an emergency such as injury or death, provide the name, address, and telephone number of the people you would like to be contacted in the event of an emergency such as injury or death.
Check the appropriate option of the kind of address your current residential address is. After ensuring that you have accomplished this form completely and accurately, sign and date this form.
When submitting through the mail, also include $10 as a fee for your replacement fee.
