Passport Forms

Passport forms are government forms eligible individuals can use to obtain a passport. Holders of expired, expiring, lost, stolen, and damaged passports may also use the applicable passport forms to request a renewal or replacement. Review all the terms and conditions the Department of State set before submitting any passport application form.

Passport forms are legal documents that eligible U.S. nationals and individuals can use to carry out passport-related transactions. These transactions include applying for a passport, renewing an existing passport, reporting a lost passport to apply for a new one, changing names or other information on a passport, and applying for a child's first passport when both parents are unable to do so personally.

Some of the uses of U.S. passport forms are obtaining the data needed to process a passport application such as the personal information of an applicant, allowing and qualifying applicants born abroad before 1960 to get limited passports without charge, enabling parents and legal guardians to represent minor applicants to apply for their child's first passport.

Passport forms are documents issued by the United States Department of State (DOS) authorized by the Secretary of State pursuant to authority granted by Congress under Title 22, Code of Federal Regulations (CFR), Part 51. They are available only at passport agencies, acceptance agents designated by DOS and U.S. embassies or consulates abroad, or through certain U.S. Government websites designated for this purpose; they are not available from the Postal Service.

Most used passport forms you can use

Form DS-11, Application for a U.S. Passport

If you are a U.S. citizen or non-U.S. national, you may use Form DS-11 to apply for a U.S. passport. Nevertheless, there are requirements before you can use it. To use Form DS-11, one of the following conditions must be true:

  • You are applying for your first U.S. passport.
  • You are 16 years old or older.
  • You received your previous U.S. passport when you were under 16 years old.
  • Your previous U.S. passport was lost, stolen, or damaged.
  • Your previous U.S. passport was issued more than 15 years ago.

Form DS-82, U.S. Passport Renewal Application for Eligible Individuals

Form DS-82 is a two-page passport renewal by mail document. Eligible individuals may file it if their U.S. passport has expired or expiring. To use Form DS-82, one of the following conditions must be true; otherwise, you need to apply for a new passport.

  • Your U.S. passport is undamaged and can be submitted with your application.
  • You received your U.S. passport when you were 16 years old or older.
  • Your U.S. passport was issued in the past 15 years.
  • Your U.S. passport has your current name.

Form DS-64, Statement Regarding a Lost or Stolen U.S. Passport Book and/or Card

Losing your passport increases the risk of identity theft and fraud. Use Form DS-64 to report your lost or stolen U.S. passport book, U.S. passport card, or both. You may start the replacement process after notifying the U.S. Department of State about the status of your passport. Once the Department of State receives your Form DS-64, it will create a record in the Consular Lost and Stolen Passport System to render your lost or stolen passport invalid.

Form DS-5504, Application for a U.S. Passport — Corrections, Name Change Within 1 Year of Passport Issuance and Limited Passport Holders

While it also serves as an application form for a U.S. passport, Form DS-5504 is a special passport application form as it serves several specific purposes. Use Form DS-5504 if any of the situations apply to you:

  • You have changed your name within one year since your latest U.S. passport book, U.S. passport card, or both were issued — your U.S. passport book or U.S. passport card, or both must be less than one year old.
  • Your identifying information was printed incorrectly in your latest U.S. passport book, U.S. passport card, or both.
  • Your latest U.S. passport book was limited to two years or less for a reason other than multiple losses or a seriously damaged or mutilated passport.

Form DS-10, Birth Affidavit

The main purpose of Form DS-10 is to provide adequate evidence to prove an individual’s U.S. citizenship when certified or acceptable documents are not available, including a birth certificate, an expired passport, or a citizenship certificate. It confirms that you are a legitimate citizen of and that you were born in the United States. It serves as a secondary type of confirmation for an American citizen’s identity and citizenship.

Form DS-86, Statement of Non-Receipt of a U.S. Passport

Form DS-86 is a two-page passport document that a U.S. passport applicant uses when he or she does not receive his or her U.S. passport in the mail within a week or so after the processing of the application. It must be submitted within 90 days so that the applicant does not have to pay the fees again. When the 90 days have elapsed, the applicant will need to reapply and pay the full passport fee. Form DS-86 prevents identity theft and fraud to protect the U.S. passport holder.

Form DS-230, Application for Immigrant Visa and Alien Registration

If you wish to migrate to the United States permanently and become a U.S. resident, you must obtain an immigration visa for foreign nationals. To get an immigration visa, you must submit Form DS-230 to the U.S. Consulate. If you prefer to apply electronically, you may use Form DS-260, Immigrant Visa Electronic Application.

Form DS-5525, Statement of Exigent/Special Family Circumstances for Issuance of a U.S. Passport to a Minor Under Age 18

Use Form DS-5525 when applying for the U.S. passport of a minor whose one parent or guardian is absent and cannot be located. Provide all the required information when filling out Form DS-5525, explaining in detail the non-applying parent or guardian’s unavailability and the efforts taken to get in touch with the non-applying parent. Together with Form DS-5525, the applying parent may be required to provide documents or evidence to confirm his or her claim of exigent or special circumstance.

Form DS-3053, Statement of Consent for Special Issuance of a Passport to a Minor Under Age 16

Form DS-3053 is submitted together with Form DS-11 as evidence of parental consent to obtain a U.S. passport for a minor who is 15 years old or younger. After submitting Form DS-3053, a passport agent will determine the eligibility and acceptability of a special circumstance.
