Family forms are documents used to report information about a friend or a family member. They can be any family-related documents used for family-related purposes.
Family members or friends are often asked to complete family forms to determine the profile of a family member. They may also need to provide the following:
- Relationship or marital status
- Financial status
- Housing information
- Health history
- Citizenship or immigration status
- Criminal background
The above details are often required to determine a family member’s eligibility for benefits, including scholarships and government assistance programs, and other applications. Family members must provide information truthfully to avoid conflicts.
Family forms are also used to transfer, relieve, or acquire affairs and assets upon death or marriage, such as a Last Will and Testament, Prenuptial Agreement, and other probate forms.
Other family forms are used to vouch for a family member for loan or travel purposes, such as a Gift Affidavit and Child Travel Consent Form.
Here are some family forms you may use
Use an Affidavit of Name Change as a sworn statement to verify or testify your identity after you changed your legal name but have not made it official in your existing legal or non-legal documents or records. If you want to change your name for adoption or other purposes, you may use a Change Name Application form. If you are a woman who wishes to take your husband's family name after marriage, you may use an Affidavit of Name Change After Marriage.
Use an Adoption Information Worksheet or commonly known as an Adoption Form, to complete your adoption process. An Adoption Form is a document you may use to provide information about the child you want to adopt legally.
A Birth Declaration Template is used to create your baby's official birth certificate. Use it to specify your child's legal name, date and time of birth, weight, length, and other birth information. If you want to request your or your child's birth certificate, you may use a Request for Birth Certificate template. If you need to vouch for someone who needs to prove his or her citizenship or birth for a job or birth certificate application, use an Affidavit of Birth. You may also use an Affidavit of Identity for verification purposes.
Divorce Petition With Children
If you want to petition a divorce, but you have children with your spouse, you may use a Divorce Petition With Children to maintain joint parental responsibility after separation, including time-sharing and custody rights. If you have no children, you may use a Divorce Petition. Other divorce-related forms are Divorce Agreement and Mutual Petition for Divorce. You may use them if there is a mutual agreement between you and your spouse to file a divorce.
Without a will, you may declare yourself as an heir of a deceased person's real property using an Affidavit of Heirship. You need to prove your relationship with the decedent and list down any surviving heirs. You may also use an Affidavit of Domicile as an heir, survivor, administrator, or executor of the estate of the decedent to prove the decedent's residency. An Affidavit of Domicile will be used by a court to distribute the decedent's properties among his or her surviving heirs.
Affidavit of Financial Support
If a court or business has requested confirmation about your financial status for employment, application, and other purposes, you may issue an Affidavit of Financial Support. An Affidavit of Financial Support is a legal document that provides proof to the court or business requesting such information that you are able and willing to provide family support, as well as financial backup. It also carries certain rights and duties related to family formation and family responsibility.
Use a Pour Over Will to assign your properties to an established trust or a person you trust. It is a legal document that helps you automatically transfer your property to other family members or institutions after death.
Authorization for Foreign Travel with Minor
If you are a parent who wants to show parental awareness that your child will be traveling alone outside the United States, you may use an Authorization for Foreign Travel with Minor. It is a form usually required by most airlines for minors or children under age 15 who are traveling without their parents or legal guardians.
Use a Gift Letter if you have given money to a friend or relative to help him or her make a downpayment for a house purchase or loan. A Gift Letter is a form you use to prove that the money you gave was a gift or donation, not a loan.
A Prenuptial Agreement is a legally binding document used by a couple contemplating marriage to each other. By filing a Prenuptial Agreement, a couple agrees to certain terms and conditions regarding family properties, finances, debts, or assets. They agree that certain affairs and assets will be the responsibility of one of them, while the others will be jointly owned or shared by both when they get married. It helps avoid family strife when family assets are divided upon separation. Family assets include any assets brought into a marriage by either spouse. Another related form is a Postnuptial Agreement. It is used by a married couple who wants to clear out or establish legal and financial arrangements in the event of a divorce or separation. If the married couple wants to amend their previously filed Postnuptial Agreement, they may use a Postnuptial Agreement Amendment.
A Cohabitation Agreement is a contract signed by a couple who intends to live together without getting married. The agreement defines the rights and obligations of both parties regarding finances and properties. It has a similar effect to a Prenuptial Agreement in family law with respect to family assets and family inheritance.
