The CMS R-131, or Advance Beneficiary Notice of Noncoverage (ABN), is a form that informs Medicare beneficiaries when a service or item may not be medically necessary and therefore not covered by Medicare. It allows beneficiaries to choose whether to receive the service and pay for it themselves or decline it.
