The Nebraska Change of Address form or Form 06-104 is a document produced by the Department of Motor Vehicles (DMV) for when residents in Nebraska need to apply for a state identification card (State ID Card). Filling out the Nebraska Change of Address form is straightforward. On the boxes and spaces provided, report the information being asked accurately and honestly. In the section of the form asking ‘Yes’ or ‘No’ questions, select the corresponding box that pertains to your situation.
To begin filling out the Nebraska Change of Address form, start by stating your BIRTH DATE. Indicate first the MONTH, DAY, and then the YEAR on the boxes provided.
