What is the Eagle Scout Rank Application form?

The Eagle Scout Rank Application form is a form issued by the Boy Scouts of America (BSA). Scouts who fill out this application do so to apply for the highest rank under the BSA, the Eagle Scout. Completion and approval of this application will allow accepted scouts into the National Eagle Scouts Association, a division of the BSA composed solely of scouts who achieve the ‘Eagle’ rank.

Can I fill out the Eagle Scout Rank Application form?

If you are currently a registered Life Scout for at least six (6) months and are currently under the age of 18, or a Venturer or Sea Scout who achieved the First Class rank in a Scout troop or as a Lone Scout, then you are eligible to fill out this form and apply for the rank of Eagle Scout. Additionally, you must have acquired a minimum of 21 merit badges as well as having demonstrated leadership within your troop or crew.

You will also be asked whether you were a cub scout, a webelos scout, earned the Arrow of Light award, and completed fifth grade upon joining.

Additionally, you must provide a list of individuals who would be willing to provide recommendations on your behalf. Listed eligible individuals are your parents/guardians, religious leaders, educational authorities such as teachers, employer (if any), and two other references.

You will also be needing to write your total merit badges earned and the dates they were earned. Note that you will be required to list down a minimum of 21, but 13 of these are required for the title of Eagle Scout. These required badges are:

Next, you will be needing your list of positions earned as a Life Scout. These positions vary depending on whether you were a member of a crew, troop, or ship or were a Lone Scout.

You must have also planned a community project as well as participated in a unit leader conference.

You will also be needing the proper certifications, namely the signatures of your unit leader, unit committee chair, BSA local council, Eagle Scout board of review, and yourself. In addition to the signatures, the corresponding date of signing must also be noted.

Finally, provide the unit number of your troop, crew, ship, or Lone Scout unit.

Important Dates
In this section, you will be required to provide a series of significant dates with regards to your scouting membership.

Age Requirement Eligibility
In this section, you will be asked to write down your date of birth to prove your age. This is important because you must be under the age of 18 to apply to be an Eagle Scout.

In the appropriate space, write down your date of birth in the Month, Day, and Year format.

Requirements
This section will cover all the requirements you will be needing for your Eagle Scout application.

First, you will need to have been active in your troop, crew, or ship for at least six months as a Life Scout. Do this by indicating the date of your Life Scout board of review.

Next, you will be asked to indicate the names, addresses, telephone numbers, and email of people who would be willing to recommend you for the rank of Eagle Scout. Listed eligible individuals are your parents/guardians, religious leaders, educational authorities such as teachers, employer (if any), and two other references.

You will then be needing to list at least 21 of your earned merit badges. Of these 21 badges, 13 are required for you to have to be promoted to the rank of Eagle Scout. These required badges are:

Note that if given the choice to write down one of multiple badges, you can only choose to write one in the appropriate section and may place the other badge(s) in the spaces asking for your other badges.

You must also write down the date the badge was earned as well as the unit number.

If you have an alternative badge in place of an Eagle required badge, you must also attach the Application for Alternative Eagle Scout Rank Merit Badges form.

You must also list down one or more of the following positions of responsibility. You may only list down positions served after the Life Scout board of review date.

Scout troop: Patrol leader, assistant senior patrol leader, senior patrol leader, troop guide, Order of the Arrow troop representative, den chief, scribe, librarian, historian, quartermaster, junior assistant Scoutmaster, chaplain aide, instructor, webmaster, and outdoor ethics guide.

Venturing crew: President, vice president, secretary, historian, den chief, treasurer, guide, quartermaster, chaplain aide, and outdoor ethics guide

Sea Scout ship: Boatswain, boatswain’s mate, pursuer, yeoman, storekeeper, crew leader, media specialist, specialist, den chief, chaplain aide.

Lone Scout: Leadership responsibility in your school, religious organization, or club, or elsewhere in your community.

You will need to indicate as well the “From” and “To” dates of each position as well as the date of your Life Scout board of review.

Next, you will have to provide proof that you headed a service project to help any religious institution, school, or community other than the BSA. Write down the name of the project, the total hours devoted to the project, the date it was held, and the date it was finished.

You will then certify all this information by affixing your signature, telephone number, and the date of signing.

You must also have the approval of your unit leader and unit committee chair, and they must also affix their signature, telephone number, and the date of signing.

Finally, you will need BSA local council verification, and a representative can note this application by once again signing their signature, telephone number, and the date of signing.

The last requirement of this form is the completed board of review for the Eagle Scout rank. For this requirement, you will be needing the signature of the Eagle Scout board of review chair and that of the council/district board representative, if applicable.

To approve this application, the Scout executive can affix their name and the date of signing.

Once everything has been completed, congratulations, you have successfully completed the Eagle Scout Rank Application form.
